My suggestion would be to implement 'Facebook' style image tagging.
I've seen a similar open source system implemented for gallery, which is demoed here (http://netbear.se/imagetags_demo/main.php?g2_itemId=21) and available for download (GNU) here (http://files.netbear.se/imagetags-1.1.1-dev.zip).
In essence, the system would allow you to select an area of an image, then associate it with a user. The searchable user list could include all forum members and any user tagged in an image would be sent a PM telling them they've been tagged (perhaps an option in usercp to turn this off).
Additionally, there would be a list of non-members who can be added by those who have appropriate permissions - this would allow tagging of celebrities (or in my case rugby players).
A field in the description area would show all users tagged in the image, with links to other images in which the user is tagged, and to the user's member page.
